Uses and Disclosures of Your Health Information
We may use or disclose your health information in the following ways:
TreatmentWe may use and disclose your PHI to provide, coordinate, or manage your healthcare. This includes communication with other providers, specialists, therapists, pharmacies, and labs. For example, we may disclose your PHI when coordinating prescriptions, referrals, or lab testing.
Payment
We may use and disclose your PHI to bill and collect payment from you, insurance companies, or other third-party payers. This may include eligibility checks, claim submissions, prior authorizations, or appeals.
Healthcare Operations
We may use and disclose your PHI for internal operations such as quality improvement, staff training, credentialing, accreditation, and compliance audits.
Communications
We may contact you by phone, email, or text to confirm appointments, provide instructions, or share practice updates.

Kartra – For secure patient communications and email marketing
Spruce Health – For secure messaging and telemedicine
Optimantra – For EMR, scheduling, intake, and documentation
BodySite – For remote health tracking and digital care plans
Gmail – Used in HIPAA-compliant configuration under BAA Appointment Reminders and Follow-Up
We may use your information to remind you of appointments or provide post-care instructions via text, phone, or email.
Others Involved in Your Care
We may disclose your PHI to family or close friends involved in your care if you verbally approve or in emergency circumstances.
Disclosures Required or Permitted by Law

Breach NotificationIn the event of a breach of unsecured PHI, we will notify you in accordance with federal law, without unreasonable delay and no later than 60 days from discovery.
Your Rights Regarding Your Protected Health Information
Access: You have the right to inspect and obtain a copy of your PHI. Requests must be in writing.
Amendment:
You may request corrections to your record. We may deny requests if information is complete or accurate.
Accounting of Disclosures: You may request a list of disclosures made in the past 6 years, excluding those for treatment, payment, operations, or authorized releases. Fees may apply for multiple requests.Restrictions: You may request a restriction on how your PHI is used or disclosed. We are not required to agree unless the disclosure is to a health plan and you have paid out-of-pocket in full.
Confidential Communications: You may request we contact you at a specific number, address, or method.
Paper Copy: You may request a physical copy of this notice at any time.
Authorization for Other Uses
Uses or disclosures of your PHI not covered by this notice or permitted by law will only be made with your written authorization. You may revoke that authorization at any time, in writing.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our office or the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
